Good checklist. Would probably ask a couple of food based questions @bakindoki - what’s the normal feeding routine, what do you do if they go off their food, any back up solutions, is there enough food/meds for the full sit Etc Also, maybe a question or two on what would you like us to use/not use in the fridge, pantry etc for human food or not 
Are pets spayed/neutered?

In addition as a sitter I always want to know what arrangements the homeowner has in place to pay for emergency vet treatment if it was needed during the sit.
